What exactly did NHTSA open, and why is it different from a recall?

EA26005 is a remedy-effectiveness investigation: it takes a repair that has already been performed on most of a 597,571-vehicle fleet and asks whether it worked. That is a rarer and more consequential thing than a normal defect probe, because the usual consumer advice — "check for open recalls, get them done" — has already been followed by the owners now complaining.

The sequence on NHTSA's own record:

  1. 2025-04-24 — GM files recall 25V-274 for engine failure in L87 engines built 2021-03-01 through 2024-05-31, citing "a supplier manufacturing and quality issue" as root cause.

How many engines actually failed after being "fixed"?

499, by NHTSA's count — but GM's own count is 6,953. Both numbers come from the same ODI opening summary, and the 14-fold gap is the most striking thing in the document. Do not read that table as "the oil change is 18× worse than a new engine."
